the child's local authority for performance (entertainment) and chaperone licenses (where the child or chaperone reside), OR:-
the employer's local authority for part-time work (place of the child's employment)
Local authority procedures may differ from area to area and individual authorities may prefer that applications for:- employment permits, performance licenses or chaperone licenses are submitted on their own authority's forms. Others may not, and are happy to accept different local authority forms. If in doubt check with the relevant local authority - item 1 or 2 above.
